District Certification of Student Eligibility

You must complete the following 2 forms and present them to the community school for every child to be registered or re-registered:

 Registration Form

 Form #101 (STUDENT ENTRY REGISTRATION-yellow)

 Residency/Domicile Form

 Form #102 (RESIDENCY/DOMICILE - tan)

Circumstance/Situation

Forms to be Completed

 The student’s parents own the house and the student and parents are domiciled/reside at this location.

 The student’s parents lease an apartment or a house and the student and parents are domiciled at this location.

Form #114 (LANDLORD RENTAL AGREEMENT)

Additional forms may be required based on your particular residential arrangements. The preceding forms ONLY have to be filled out if your child meets the circumstance/situation below.

 If the student is living with a parent or guardian who is a non-rent-paying tenant living at the address given on the registration form.

Form #109 (NON-RENT PAYING Parent/Guardian-blue)

Form #110 (FOR THE LANDLORD-pink)

 Living with a person, other than the parent or guardian, who is domiciled in the district and is supporting the student without compensation,

 as if the student were his or her own child,

 because the parent cannot support the child

 due to family or economic hardship.

 If the student is living with a parent or guardian temporarily residing within the district.

(TEMPORARY RESIDENT)

Form #113 (INTERIM AFFIDAVIT-buff)

 If the student is living with a person other than a parent/guardian within the district and does not fall within the “Affidavit” definition.

(SPECIAL CIRCUMSTANCES)

 (Special circumstances include homelessness,

 court ordered placement, DYFS intervention,

 parent activated by National Guard)
